Office of the Registrar - Diploma Application Form

To ensure that your diploma is ordered with the name that you prefer, please complete your diploma application form by Mid Semester of the semester that you plan to graduate. If your diploma application form is not submitted to the Office of the Registrar by Mid Semester, then the Graduation Section will order your diploma with the name that appears on your student record in Tiger i.

If the Office of the Registrar receives a diploma application form after Mid Semester and the student wants their name to appear on their diploma differently than our office has previously ordered the diploma, then the student can request a corrected diploma for an additional charge of $20.00.

If you have checked the confidentiality flag in Tiger i, you must sign and date your diploma application form in order to release your name to be printed in the commencement program and give permission for your name to be announced from the platform at graduation. If you have a confidentiality flag on your record and our office does not have a signed release from you, our office will contact you through your Auburn e-mail address to inform you about this needed release information.
